Santa Fe Home Care is proud to offer quality care services to individuals in El Paso. If you or a loved one are interested in enrolling in one of our programs, meeting eligibility requirements is mandatory.
Homemaker, Provider & PAS Services
Homemaker services, along with Personal Assistance Services (PAS) and provider services, offer comprehensive support to individuals who need help with daily living activities. Homemaker services assist with household tasks such as cleaning, cooking, laundry, and grocery shopping, ensuring a clean and comfortable environment. PAS provides hands-on care, including help with personal hygiene, mobility, medication reminders, and other daily activities essential for individuals with disabilities, seniors, or those recovering from illness. Provider services often coordinate these efforts, ensuring that care is personalized and adapted to each patient’s unique needs. These services promote independence, enhance quality of life, and ensure that patients can thrive in their own homes with the right level of care and support.
Eligibility Criteria
To join the PACE Program, prospective participants must meet the following requirements:
- Be 55 years of age or older;
- Be certified by the state as requiring nursing home-level care;
- Reside within Santa Fe Home Care’s service area (check our Service Area Map for details)
- Be able to live safely in the community.
Santa Fe Home Care supports applicants throughout the application process, but the State of Texas ultimately makes the decision on application approval and enrollment.
Steps to Enroll in the PACE Program
Enrollment in the PACE Program is voluntary, and applying is free. Here’s a summary of the process:
- Interested individuals request enrollment information and assistance.
- A Santa Fe Home Care nurse visits the applicant's home to provide additional information, answer questions, and verify eligibility. Applicants and their families are also welcome to tour the Santa Fe Home Care health center.
- If the applicant is eligible, the process moves forward with a physical evaluation, a review of medical history, and a financial assessment.
- The completed application is submitted to the State of Texas for review, which can take approximately 30-60 days to reach a decision.
- Once approved, Santa Fe Home Care collaborates with the participant and their family to finalize the enrollment into the PACE Program.
PACE Program Costs
Submitting an application for the PACE Program is free. For participants eligible for both Medicare and Medicaid, all services provided by Santa Fe Home Care and PACE are fully covered, with no copays or out-of-pocket costs. Participants may be required to pay a premium under the following conditions:
- If they are not eligible for Medicaid.
- If they are only eligible for Medicare but not Medicaid, the participant will need to pay premiums equivalent to both Medicare and Medicaid.
All necessary healthcare services, as determined by the Interdisciplinary Team, are covered under the PACE Program. Participants are responsible for any unauthorized services that fall outside the program’s scope. Santa Fe Home Care also provides Medicare Part D coverage for prescription medications. However, participants who choose to enroll in a separate Medicare Advantage Plan will no longer be eligible for the PACE Program.
